How to Request for Change of Phone Number on CAGD E-PaySlip

Do you wish to change your phone Number on the CAGD E-payslip? This option is not available on the Portal.

This is to ensure that scammers do not get easy access to your portal to:

1. Generate Mandate Form
2. Change your Password
3. Check your affordability etc.

You will need a template to submit or change your new phone number with the old one.

Personal Details include:

1. Full Name
2. Staff ID
3. SSNIT Number
4. Ghana Card Number (with a photocopy attached; front and back)
5. New Phone Number
6. Old Phone Number

You will also need your Validator Details

1. Full Name of Validator
2. Staff ID of validator
3. Validator’s Phone Number
4. His/Her Signature
